Private Contract Services

Home Physical Therapy for Seniors is a private fee-for-service practice.  Our licensed physical therapists do not accept payment from third party payers.  Under our private contract, you willfully refuse to authorize the submission of a bill to Medicare, workers compensation or any other commercial health insurance provider for any items or services furnished by Home Physical Therapy for Seniors.

The limits on what Home Physical Therapy for Seniors may collect from you is comparable to charges determined by Medicare and other commercial health insurance providers for the same services, despite the added advantage of receiving care in your own home.

Flat Rate for Services (to be paid at date of service):

Initial Evaluation and Treatment:  $100.00 (one-time fee)

Follow-up treatment visits:  $75.00
