To request an appointment, a prescription for physical therapy from your physician is required. If you do not already have one, we can contact your doctor for you to request one.

To find out if you are a candidate for physical therapy, call us toll free at 866-4-HOME-PT (or 401-536-4117) for a free consultation with a licensed physical therapist. 

An appointment for your initial evaluation or treatment visit can be requested online by completing and submitting the form below. A licensed physical therapist will respond to your request within 24 hours. Please do not use this form if you have an urgent medical problem or to re-schedule an existing appointment.
